Need to replace my Remote with the head set attached

I Am in need of a replacement remote with the head set attached.  I am having trouble finding one. Any suggestions?

My remote is wearing out cannot find a replacement remote with the head phone on it

The original Voice Remote Pro has a headphone jack:
