Disengage voice annoucement on remote when changing volume and channels

I accidentally engaged voice assistant that resulted in announcing volume and channel changes.  How do I shut this off.  Where is located under Settings?

Not sure about the volume announcements (does not occur on my devices), but the Screen Reader under 
"Accessibility" will announce channel changes and might be your culprit.

Settings/Accessibility/Screen Reader [Off]

You can toggle this on/off by pressing the asterick (*) button on your Roku remote (or mobile app) 4 times.

You can disable this shortcut ability under Settings/Accessibility/Shortcut [Off] <-- this will disable the 4x asterick shortcut/toggle so you can't inadvertently trigger it again.
