Don't Know why I have not seen this fix anywhere on line.
But here is the fix.
Download the remote control Roku app on your cell phone.
In the top right corner you will find setting
Scroll down to FIX THE SOUND DELAY WITH YOUR PHONE.
This is unbelivable that Roku has a fix that works and nobody knows about it

After trying all these fixes the Roku Remote App fix finally worked. The weird part is that the audio wasn't out of sync on all of the shows or the commercials during said show.
Solved audio sync issue by changing Digital Output Format from Auto to Stereo.
Go to Settings: Audio: Digital Ouput Format: check mark Stereo.
This will solve all Audio delay and syncing issues.
I tried the Roku "fixes" for audio sync, nothing worked. None of my other TVs, or any TV I've had in this room, have this problem. My Roku set doesn't have an option in Settings? By the time I put the tv up it was too late for a return, so going to the Transfer Station to trash the thing. Apparently Roku doesn't care. Just ordered a TCL.
Tried the in-app remote control synch adjustment tool in Settings. Better, but not 100%. Surprising given the number of strobe/sound pulses used in the synch adjustment process.
Thats really not a solution... That's like saying the car is faulty and the fix is walking.
There is no sync adjustment in my Roku TV Audio settings. TV speakers are synced; digital output, if selected to Stereo (PCM), is synced; but if digital output is selected to Custom Dolby, audio is not synced. The Roku Remote app sync procedure completes successfully but doesn't seem to do anything.

I have a Westinghouse Roku TV model #WR58UE4100.
The problem is on all channels and is the same on all channels.
I either use an over-the-air antenna or stream via WiFi. I either use the Roku TV speakers or an Onkyo receiver connected via the Roku TV optical SPDIF output with the TV speakers muted. I do not use eARC. The Onkyo receiver does not support ARC. The Onkyo receiver displays the audio format it is receiving via optical SPDIF.
The Roku TV "Audio output" is configured for "TV speakers" (the optical output is always on).
With the Roku TV "Digital output format" set to "Stereo", the Onkyo receiver indicates "PCM fs: 48 kHz" format and there is no audio sync issue.
With the Roku TV "Digital output format" set to "Custom - Dolby Digital", the Onkyo receiver indicates "Dolby D 3/2.1" or "Dolby D 2/0" depending on the channel and there is a noticeable audio sync issue.
As I wrote before, there is no sync adjustment in the Roku TV Audio settings and the Roku Remote smartphone app sync procedure completes successfully but doesn't seem to do anything.
I am considering buying an eARC-to-optical audio extractor and setting the "Digital output format" to "Passthrough", hoping this will eliminate the audio sync issue. And, also set the Roku TV "Audio output" to "Auto" hoping the Roku TV will automatically turn off the TV speakers if the audio extractor is powered-up and turn on the Roku TV speakers if the audio extractor is off.
Please let me know your thoughts and/or solutions.

It is a great idea to consider purchasing an eARC-to-optical extractor that would bypass the Roku TV’s SPDIF processor and provide a cleaner, more direct path for Dolby Digital. eARC handles audio formats with lower latency and less compression. Then, set the Digital output format to Passthrough as it sends raw audio data to the extractor, minimizing internal processing and possibly reducing sync issues.
