Only have a barcode screen on setup

I tried to setup on my Samsung tv on my new Roku and it failed. When I tried the second time all I got was the barcode screen. 
I went into my Roku account and removed all the Roku’s associated with that room and started all over again. 
Got the barcode screen again. All the while uninstalling and reinstalling the Roku. 
Just the barcode screen again. 
Why doesn’t Roku have phone support?

What's a "barcode screen"?  Can you post a picture?  Do you mean the QR code that you'd scan with your phone's camera?  Maybe start here:

It’s the QR code that takes you to the Roku site that tells you how to setup your Roku. Same as what you get in the package.
