The Roku Ultra and Streaming Stick devices come with a Roku voice remote, which is RF-based. This remote and its voice command features can be controlled using Wi-Fi, so resetting it may resolve the issue of randomly changing volumes, as the volumes can only be controlled using the remote.
In some cases, directly rebooting the Roku player may also resolve this, as rebooting clears the cache. I suggested troubleshooting steps to help different users affected by this resolve their issues, as users have different Roku devices, which may or may not work for them.
We are currently monitoring this case to help determine the cause and find the best resolution possible.

SOLVED!!!!! Finally figured out how to fix this ongoing Roku Issue that is clearly getting No Support...
Step 1 - Turn off your Roku device
Step 2 - Disconnect all cables and power supply from the Roku
Step 3 - Throw the Roku into the Trash.
Step 4 - Reconnect your Streaming Apps through the built in and FREE streaming service you can get from your Smart TV.
I also have an Ultra and have the same problem. When the volume either drops or gets very loud, I pause streaming, back up on frame, and then play the volume goes back to normal. The problem is so wide spread I have to think it may be a hardware problem. If it was software, they would have fixed it.

I have the same issue with a 3920 Roku Premier unit. It is using version 13.1.4 Build 1512-91 software.
I have found that a audible click or pop will be heard as the volume changes. I then need to do a Pause, the click the left arrow next to the Home button which takes you to the screen describing the currently selected program with the option to "Resume" which I then click on it. Most of the time the volume is correct, but sometimes I have to do it a second time.
The remote has nothing to do with it since there are no volume controls on it and it is not a voice enabled remote. The original TV remote is used to control the volume and it only gets used to turn the TV on/off.
Obviously Roku has no idea how to fix this problem, I have 3 roku ultras and all 3 are fully up to date build 1512, all 3 have this volume problem, I do not see the settings mentioned as solutions in the ultra settings menu. can anybody tell me if this problem is resolved in the new 2024 version ?
Me too for at least two years with my streaming stick+ 4K devices. Have to change to another channel within YouTube TV and go back to channel I was watching to get volume to return to previous level. Very frustrating.
I've found that doing the go back a few seconds button (counterclockwise curved arrow) gets the volume back to where it should be.
