1 of 4 lights dead on arrival

I need to return it and Roku sent me an RMA number but not a return shipping label.

I also wonder if I need to return just the one dead bulb or all four even if three are working fine.

I have waited several days. There is no chat option and no customer service email address or phone number.

Any suggestions?
