thank you for posting. been working for hours trying to figure out.
Your solutions do not work. My screen reader is disabled but is still on. I powered down the Roku and the screen reader is still on though disabled. The normal screen reader controls are not working.
@Jimbob3, are you trying to say with the Roku device "powered down" (not sure what that means since only Roku TVs can be turned off) you're still hearing a voice? Some TVs have their own implementation of a "screen reader".
No, I disconnected the power cord from my Roku 4k device and plugged it back in. Turns out it was my TV. Don't know how that got turned on because that menu is about 3 levels deep in the settings.

@Jimbob3, some TVs (LG and Samsung, I think) enable it with a long press of the mute button. That may be how it got enabled.
I followed all the directions, screen reader will not shut off.
Can the Roku be reset?
No matter what I try, I cannot stop the screen reader from calling out volume levels, etc from what appears on the screen.
