Can you please provide the serial number of your Roku device from Settings>System>About? We sincerely appreciate your patience and we are working on ways to better improve this process.

For verification, have you checked if you enter zero instead of the letter O at https://my.roku.com/HDMI?

Hi, This is still an issue - please fix your website form as this is clearly an ongoing issue.
I've used both the SN on the stick as suggested on the form and the serial number in the settings that I had to set up on another device since the stick will not fit on my TV without the extension. I've tried to fill out the form multiple times, in different browsers, on different devices, and it will never let me continue past the initial form.

I tried both:
255O5H215895 (the letter O)
and
25505H215895 (the numeral 0)
both do not work - I don’t even get a prompt.
Because I can’t send a photo but this is a serial number for a “Roku Streaming Stick+“.(HD 4K HDR)
Why is this so difficult - I have 4 Roku devices, purchased over the past 10 years!
Please send me this FREE part.
