New Roku user, just setup a Roku Express HD and I’m having the same issue with some channel icons not showing, instead end empty “missing image” placeholder is there. The channels still work, they are just missing the icon.

We appreciate your report on the issue. If you haven't attempted any troubleshooting yet, please try performing a system reset on this device using your Roku remote.; navigate to Settings > System > System restart. Then try again to see if that helps.

Has this issue been resolved I have brand new Roku TV and only a few of the tiles have the icon graphics. The rest of the tiles are a generic tile.

This has to be done is settings / advance settings/ factory reset.

We recommend restarting the Roku TV system. Navigate to Settings > System > Power > System restart, then try again to see if that helps.

This is happening to me. I have done all of the recommended trouble shooting including a factory reset. Still having the problem!
