- If you can access your settings, can you tell us what the latest software version is on your Roku? (Just head over to Settings > System > About to see it.)
- Is this happening when streaming on a channel?

I have had the device for a few months and have noticed the issue 4 or 5 times in that period. The software version is 14.1. It does not happen when the device is in use. I use it mostly for AirPlay and I discover the issue when trying to AirPlay something from my iPad and it fails to connect. The device is prompting for activation (which I do via the QR code). Once activated I then have to also enter a one-off code for AirPlay connectivity, even though I have done that many times before. It’s almost as if the device has reset?

Oh, just to add it’s powered off the TV USB port. However, I assume losing power when the TV is off should not mean that it needs re-activation.

Markt1891, some TV's can't really supply enough power from their USB ports and it can cause all kinds of strange problems. Try a USB power adapter/charger from a phone/tablet that can supply at least 1 Amp (1000mA) plugged into the wall and see if it still happens.
