Roku disappeared from account and needed to be reactivate

Roku Express that has been installed and working for a year and a bit.  Went to watch tv yesterday and the device was back to defaults ready to be installed.  Looked at my Roku account and the device...

Power source issues can cause all kinds of strange problems.  If you're using the TV's USB port to power your Express, try using a USB power adapter from a phone/tablet that can supply at least 1 Amp plugged into the wall.  Roku updates in rare cases can also cause a device to basically factory reset, but there's not much you can do about that.
