Generally, have to be on the home network for the mobile app and the Roku device to communicate. When traveling away from home network, usually need a physical remote.
If your phone allows (or you also have a laptop with you), you can try establishing a hotspot connection on the phone and connecting the Roku device to the hotspot as well. Of course, you will be using your own data rather than the hospital wifi; but short of getting a physical Roku remote (or compatible universal streaming remote), may be only option for you.
