OK I fixed this at least for me., after all the "routine" stuff didn't.
Acces the secret reset menu homeX5, FFx3, rewX2
DON't choose factory reset, but scroll down to "cycle software server" and choose that.
My device loaded version 14.5 AND updated 4 apps after I choose that and all seems to be working

I was having the same issue as everyone else with the failed update and Error 004 code. I was able to resolve it by going into the secret menu and doing a soft reset. Once I did that I had 3 different updates to go through until it said there were no more. I was getting tired of the failed update attempts everyday, then having to restart again to be able to use my Roku.

ROKU Update is not working 004 error
Hi. I’m trying to update the software & I keep getting an 004 error message when the update is about 82% complete. All I have is a screen with ROKU & the time. I can’t get back to the Home Screen to access the settings. Can anyone help? Thank you so much!

Software Update will not complete and stops at 77%.
Roku attempting update, but stops when it hits 77% complete. Can't complete the software update. Any suggestions?

I reported my fix a few days ago. But here's what worked for ME.
shut the power off to the Roku. Nothing else will unfreeze things.
wait 20-30 seconds or more. then turn back on.
choose the "wait until later" option. , it should work now, but still hasn't updated.
To do RHAT, access teh secret menu
Home button five times
Fast Forward button three times,
Rewind button 2 times.
This brings up a reset menu... DO NOT select the top item "factory reset"
Scroll down to "cycle software server" and select that.
this should proceed with the update to version 14.5 and also updated 4 other apps I had on my device.

When the update fails to complete here are some things that some people have found to work for them. They may not be universally successful, but they are at least worth a try
Easiest to try, so try this first: Restart your router, even if other devices on the network are not having problems. When your router has completed its startup, try repeating your Roku setup.
If you are powering your Roku by plugging into the USB port on the TV, try powering from house power via a USB power adapter, as some TV USB ports don't supply enough power to drive a Roku reliably. If your Roku didn't come with an adapter, or if you don't have a Roku adapter handy, another USB power adapter you have from another device should work as long as the fine print on it says it outputs at least vac at a minimum of 1 amp.
Try using a different WiFi network to do the update - perhaps via a WiFi hotspot on your mobile phone or by taking it to a friend's house - and see if it will complete the startup. If successful, then redo the WiFi connection to use your intended WiFi network via Settings > Network > Set up connection. Why this works, when it does, I haven't a clue.

Update fail error code 004
An update cannot finish, an error code 004 appears then screen goes purple. I cannot access settings or any streaming.
