Free HDMI Extender. I cannot fill the form. The page have problems

I tried 3 times to fill out the form to request the HDMI extender, but the page is having problems. Please correct this so that we can make the request for the extender as you offer it. Thanks. I nee...

Please reply here with the serial number of the affected Roku stick/s. It can be found in Settings > System > About. We'll be able to further assist you from there.
