I have a 2 month old Ultra that continually needs to be reset (maybe once or twice a day). The image while streaming jumps around and pixelates, but when you hit the home screen is stable, as it should be, but unless you reset it there, you cannot go back and stream without the jumping and pixelating. It appears to be a defective device. Any remedies? How do I contact Roku for a replacement?

Have you tried switching out the HDMI cable used to connect the Roku device as well as connecting to another HDMI port to see if that helps resolve the issue?

We have not done that yet, but I might try.
It appears that it may only be happening with amazon prime. WE have removed and reinstalled amazon and await the results.

When removing the channel, make sure that you restart your device before adding the channel back again, specifically in that order.
