I think here is a related issue (4 months old, no answers):

Video player fails to create after exit with a Home button (I'm experiencing the same issue, probably because my whole scene is added to m.global).
When I exit normally with a back-back-back-back - everything is ok.

So I guess video player remains in memory and Roku Firmware does not allow to create a second instance of it.

Also this issue with Roku Ads Framework could be related:

Behind the scenes - if i remember - the video player in Roku is a singleton. I.e. the different ro- or sg- components are just facades that talk to a single "guy". 

It would seem some ways of exiting scenographic channels leave said player in "wedgied" state.
