You never said how you got out of it. Did hitting the Home button return you to the main menu? Did it simply time out? Did you have to unplug the Roku from power? How did you exit?
Also, please verify the name of the app. There is no app called "Casino Slots" in the Channel Store. This thread is for that, and either it was renamed, or it was a non-certified app and was removed. So, what is the actual name of the app? Unless we have that, we can't attempt to replicate it.
Yes, there are 27 different apps with "Slots" in the name, so we need to know which is the one you're using.

The description says "SERVICE MAY REQUIRE ADDITIONAL FEES" which usually means you have to subscribe to a service.
You said you "paid for the game" meaning that you have a subscription? Do you see it on your Roku Subscriptions page? If so, and the game doesn't work, you should work with Jeremiah-Roku or one of the other billing representatives. Although they will probably suggest you contact the PlayWorks Digital developers. They will be the ones to give you proper advice, however.
