It was factory reset
Then you will have to re-select and and enter your Wi-Fi password. (This happens after you select your language, if I recall.)
The recent change is that Xfinity had to provide a special code to get my internet/email working. After the special code, no Roku tv. Tom
Sorry, but I don't know what special code means.
New Wi-Fi SSID or password?
New Xfinity account password?
Something else?
If something else, does it have a more specific name or can you describe where the code was used?

Error 016

Welcome to the Roku Community!
Error code 016 happens when the network connection is lost while launching a channel. To fix it, restart your Roku to clear its memory via Settings > System > System restart (for a Roku TV, it's Settings > System > Power > System restart).
Again, sometimes that's all it takes; if not, keep going. Find and connect to your network via Settings> Network > Set up connection. Let us know if there are any changes.
