Audio reader will not turn off even after all recommendations

I cannot turn off audio telling me of volume and channel changes.  Have tried all recommendations and settings.  Going nuts.  Help!!!!

Did you press the Options button [*] four times?

Or go to Settings > Accessibility > Screen reader > Off

@Gaucho1, is this a Roku TV or a Roku device connected to a TV?
