Simple Video Player Crashing

So I'm trying to create a channel on roku with the Simple Video Player app. I'm using Wowza with Wirecast as my setup. My goal is to use the nDVR feature that Wowza has, and I have it up and running as far as I can tell. It's showing up on any computer I have on the network. Now I'm trying to have the Roku box pick it up. I've got this to work before, but that was a year ago. When I install and launch the channel it's fine. But when I click play the channel basically just hangs for a few seconds and then goes back to the main menu. I feel like I've followed the same steps to set this up and I even wrote down directions for myself a year ago so I wouldn't forget. I'm clueless. I don't know what's changed because it should technically work. Anyone have this issue? Is there a better way to do this? I'm quite sure it's just a problem of the URL or something, because I've tried:
http://10.10.2.84:1935/dvr/myStream/playlist.m3u8?DVR

As well as without the ?DVR at the end.

Try adding
srt="" 
after the title= "CPC Testy Streamy" line.

Turns out the solution was something similar to that. There was code further down that was trying to reference a srt subtitle file that wasn't there and it was causing it to crash or do something terrible. I just commented it to look like this:
