I am having the exact same issues and bought the soundbar along with the subwoofer and speakers in September. Even though I love the surround site, the streaming through the Soundbar is most often problematic with poor performance of freezing, rebooting, etc.
My Model is a 9102R - Roku Streambar with a software versions of 12.5.0, build 4178-95. Not sure if you would also need the Serial Number?
