Roku shutting off and keeps asking to activate

After setup I select an app and roku shuts off turns back on and asks for the activation process all over again. Such as language country and email. Email send she activation link and activates then when its done it shuts off and I have to do it all over again. Anybody else have this issue? And how can it be fixed?

The first question is are you using the supplied power adapter and not your TV's USB port?
