My family likes to talk during movies and shows, so I'm looking for a way to use the private listening feature on the app, but still need the audio to come through the TV. Does the app or any 3rd party app do this?
Why not? This seems like a simple coding fix where audio can play of both the app and TV simultaneously.
the original post makes a clear point why you’d want to do this. It would also be useful for people who are hard of hearing- so they can increase the volume on headphones while leaving the tv at a lower volume for others.
I also want it so I can use my noise canceling headphones connected via Bluetooth on my iPhone while watching a movie with my family.

I wonder if adding this feature will cause echoing. E.g., when using my Roku Streambar, I am now (and this is a new issue) getting sound through both my TV and Streambar and extremely annoying echoing that comes with it.
I'm posting this with the hope that Roku will not adopt changes that address the wish list for one of its customers but creates an issue for others.
The suggestion I made would not result in echoing since one device is meant for the room (tv speaker/soundbar) and the other device is personal (headphones)- therefore only 1 audio source can be heard by a person, completely eliminating the possibility for echo. This is a suggestion for any user who is trying to watch roku with another person- this is not for a single person but rather the majority of users.
Your issue with echoing seems to be due to your soundbar setup. I use the optical cable and can use both my soundbar and TV speakers together with zero echo.
