I'm also having this same problem with the www.roku.com/HDMI web page. I fill out all my information and click CONTINUE and then nothing. The page doesn't advance to another, doesn't tell me that it will be sent, doesn't tell me I'm missing information...just nothing.

@Jim1957, did you try the serial number that's shown under Settings->System->About?
The serial number in my settings is YF0024566245. I would assume the 00 is not letters. Can you please call me 812-202-1241. I would like to use my Roku but I can not with the extension. It has been more than two weeks I have been seeking help. Is there someone I can get help ordering this? Please.
I have tried the serial number on the stick itself and the side of the box (They are the same serial number). I have tried the serial number in Settings - System - About (a different number). None work to request the free HDMI extender. After entering my information nothing happens...absolutely nothing.
What can I do? I'm not the only one with this issue. Maybe I will order a couple from Amazon or somewhere, just to be able to use my Roku, that that is poor customer service.

When inputting the serial number, we suggest making sure to put zeros instead of the letter O. Please note as well that serial numbers usually start with "YH". To know your device's exact serial number, navigate to Settings > System > About.
If still no luck, reply here with the serial number of the affected Roku stick for further assistance.
