Premiere+ Remote Not Pairing after Factory Reset

I was recently given a Roku Premiere+, which came with a paired remote.  I was able to use the remote without any issues to navigate menus; however, due to networking issues, I performed a factory reset on the device.  After the reset, the remote is no longer paired; I'm unable to navigate the setup menu when I start up the device.  When I follow the on-screen instructions to pair the remote, the green light on the back of the remote blinks for ~60 seconds, but it never pairs with the Roku.

I've tried multiple power cycles of the device and remote, and put fresh batteries in the remote, to no avail.  I downloaded the Roku app for my phone, but that seems to require the Roku to be network-connected, so I'm currently unable to use my device.
