Roku Streaming Stick+ 4K crashing

My streaming stick+ is crashing. Sometimes it goes to a black screen for a few seconds, then returns. I lose video and audio during this time -- and the TV goes black (e.g. lost HDMI signal). When it returns, the stream I was watching has continued in the background, and I have to skip back if possible.

This happens in Netflix, Amazon Prime Video, and YouTubeTV.

The TV is a 2016 Samsung 1080p.

I have the same model of Roku on another TV with no playback issues.
