Premiere+ Stick Black Screen and no sound

We've been using our Roku Premiere+ for about 9 months now with no issues, but suddenly today we only get a black screen and no audio.  Nothing else has changed in our setup, so I'm thinking it must have been a new update pushed out by Roku that broke it.  Here's what i've tried:

- Unplugging/Plugging it back in.  This shows the bouncing roku screen, then just goes to black screen.

- Using a different HDMI port and different HDMI cables.  Didn't help

- Held the reset button on the back to factory reset it.  This actually got me to the setup screens, but when I got to the point to auto detect display type it  flashes to black screen again and stays like that forever.
