Proper code format for DFP Doubleclick for Publishers

I've tried about 20 different combinations of Google DFP tags in adUrl: (See below for a handful of examples)

Everything I try shows up as Unfilled impressions in the DFP report. Can anyone give me a sample that I can just plug my ID into that will work?
