I can't test my app on roku 3

Hello!
I have roku 3 device. One month ago I could deploy my test app by eclipse(or by Development Application Installer) to my device. 
Now, it's not working. I can ping roku device, but I can't open Development Application Installer page in my browser. (I tried ports 8080 and 8085)
I follow the instructions at here. Paragraph 2 - impasse
Otherwise the device works well, I tried to reset to default settings. Nothing!
Can you help me? How I can test my apps now?

Are you sure the application installer is enabled? Have you violated the TOS in some way that you can no longer enable the application installer?

Yes. Application Installer enabled! 
When I press "Home Home Home Up Up Right Left Right Left Right" then I can see that Roku offers disable installer and restart.

What is the TOS?

Terms of Service.  If you have in the past tried side-loading one of a few channels that gives access to pirate streams, Roku will disable your ability to enable the application installer.  Read this thread:.
