Access code returns an error for beta channel

My beta app overview has checkmarks on everything but monitization. When I attempt to use my access code at https://my.roku.com/account/add?channel=my-code, the form returns an error without a message.

In Dev Tools the network shows a 404 on the request, however, the code is valid.
