Unable to add PLEX

I just installed my Roku, and am unable to add the PLEX channel, although no reason is given within the interface.   Has anyone else experienced problems with installing PLEX or, if you had already installed it, does it still work for you?

What is the model number of your Roku from Settings->System->About?  Also, what Software Version is shown there?
