Obviously by the number of complaints, I'm not alone in this frustration.
Web page is now working. Request went through and now I wait 3-5 days and hopefully I receive my extender cables.

Form worked for me today BUT only by using serial number as on back of stick. This is the device ID in settings. I'll see what happens in 3-5 days when delivery is due.
