Roku Indoor Smart Plug SE (PS1000X) Status Lights No Longer Light Up

I purchased the Smart Plug a few weeks ago and it quit working after originally working for a few weeks to light the lamp. 

The Status LED no longer lights up no matter what I do. 

I tried moving to a different outlet where another Smart Plug is working and no matter how long I hold the reset button on the side no lights appear. I also confirmed no power flows throw the plug to the lamp.

I have 5 other Roku Smart Plugs in my house that are working correctly in the Roku App as expected.
