Roku developer support form is broken

Hi, I am trying to report an issue we found with firmware 7.6.0, but the online contact form is rejecting my submission: https://developer.roku.com/en-ca/contact

With all fields filled out appropriately, the last field always turns red and says that it is blank, which prevents submission.
