As noted, Roku does not write apps other than The Roku Channel. If you want an X app, you will need to contact X.

The x app is already installed on Roku.
The original poster asked stated:
I get a request for my "Provider ID" which is all numbers. Where do I find this?
This is the same issue I am having. The app shows no website or phone number to call. This is very concerning as it may be malware and have access to my network...
ill do a factory reset just in case.

There should be no need for a factory reset. Just uninstall it. This isn't like Android or Windows. Roku is a unique OS.
