    It doesn't sound like you've tried anything with the USB flash drive.  I'd try taking it out, reformatting it in a computer, and then put it back in and let the TV reformat again.  If that doesn't work, try a different USB flash drive.  Maybe a different brand.

Yes. In the Chicago market the following channels have an issue with repeating. 
32-1, 2.1, 48.3, 5.1, 5.2, 7.1, 9.1, 9.2, 9.4, 11.1, 11.2, 11.3, and 26.6. It’s essentially every local channel in the market. There are more digital channels in the market, but they are not channels that I watch.
