Are you using one of the screensavers (like City Stroll) that have little advertisements floating by? If so, pressing Play/Pause (and possibly OK) on your remote will launch whatever it's advertising. It's possible that you're inadvertently pressing a button or your Roku device is receiving a stray signal that it's interpreting as one of those buttons. There's other advertisements on the Home Screen that will do the same thing.
If you're using one of those screensavers, try using another or disabling it. To rule out the other possibility, make sure you're not pressing any buttons when you pick up the remote and you can go as far as covering the 4802 with a box so there's no way any IR signal can reach it.

Using the fish aquarium screensaver, same as before the update.
Example 1, there was no human interaction, no one in the room, i could see it happen from the doorway outside. so no inadvertently hitting any buttons. suppose a ghost maybe done it.
Example 2: only home button on Roku remote was pressed, or only input selection on TV.

I've been unable to duplicate the issue after deleting Roku Channel. i never use it anyway.

CONFIRMED! Roku Channel is the absolutely the culprit.
Just now, I hit Home Button 1 time, Roku wakes the TV and then installed Roku Channel without asking, immediately launched into playing Severance S1 E1 without any input additional input from the user.This is the 3rd time it has started some R or MA rated tv show on it's own, what if kids were around Roku?
